Failure to return your completed forms to the parish office will mean that you cannot take up or continue with your voluntary work until the forms are received.&nbsp;<\/p>\n\n\n\n<p><strong>Volunteer&#8217;s Code of Conduct\u00a0<a href=\"\/new\/wp-content\/uploads\/2019\/12\/CodeOfConduct.pdf\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">Download this form<\/a><\/strong><\/p>\n\n\n\n<p>In response to the Royal Commission into Institutional child sexual abuse, the Archdiocese of Sydney has set up a Safeguarding and Ministerial Integrity Office which has developed protocol to create a safe and nurturing environment for children and vulnerable people. &nbsp;The Archdiocese has now mandated that every adult volunteer within each of its parishes MUST sign a Code of Conduct.<\/p>\n\n\n\n<p><strong>Part F2 Working with Volunteers Form<\/strong>\u00a0<strong>\u00a0<a href=\"\/new\/wp-content\/uploads\/2019\/12\/Part_F2_Working_With_Volunteers_St_Chris_Holsworthy.pdf\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">Download this form<\/a><\/strong><\/p>\n\n\n\n<p>This form (relating to Workplace, Health and Safety) is required to be completed by every volunteer at the start of each new calendar year. &nbsp;<\/p>\n\n\n\n<p>Please download and complete this form and return it to the parish office at your earliest convenience OR no later than 1 March each year.<\/p>\n\n\n\n<p><strong>NOTE:<\/strong>&nbsp;If you volunteer across a number of ministries &#8211; for example: sing in a choir, Commentator and an Extraordinary Eucharistic Minister, please check with the parish office first if you are required to complete a separate form for each ministry.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Forms Planned Giving Application\/Renewal Form\u00a0Download this form Please complete this form to: Join the Parish Planned Giving program Change contribution method from envelopes to credit card Update your credit card details Parish Census Form\u00a0Download this form Please complete this form: For new parishioners to be entered on our Parish register.
